Advance Care Planning isn’t just paperwork. It’s about making sure your voice, your values, and your wishes are honored. It protects you and removes uncertainty for the people who love you.
Whether you’re planning ahead, navigating a new diagnosis, or supporting a loved one through illness, this work creates clarity and calm in moments that often feel uncertain.
What You Receive
In our session together, I will help you:
- Clarify your medical, emotional, and spiritual care preferences
- Identify who will speak on your behalf if needed
- Begin or update your Advance Directive using the Five Wishes framework
- Review additional Arizona documents you may need or prefer (Living Will, Healthcare Power of Attorney, POLST)
- Receive clear explanations in plain language – no legal jargon
I am a certified Five Wishes Facilitator and experienced in Arizona’s planning documentation requirements. The Five Wishes document is legally recognized in most states, including Arizona, and does not require a lawyer or physician to complete.
If you already have documents but feel unsure about them, we can review and update them together.
Investment
90 minutes (First Review Meeting) – $100
$45 / hour for follow-up or additional time

This service is open to ALL – whether you’re in your 40’s and healthy, facing a terminal illness, or simply entering a stage of your life where you are ready for that peace of mind.
Please note that persons with Alzheimer’s disease or dementia do not have to be excluded from advance care planning – it may simply look different! We can talk about what’s appropriate for your situation.
